If you are the designated point person or pro bono lead for your firm or organization, please fill out this survey on behalf of your firm. You may also submit the form as an individual. You will be able to return to update your preferences in the future.

The survey will allow us to track and efficiently route Project Applicants to your teams for service. As you fill out the survey, consider that Applicants are not screened for income or conflicts, the Project is designed to replicate the in-person walk-in model in order to offer assistance as quickly and efficiently as possible to the many small businesses suffering during the current crises.
